Adult Forms

• Adult Authorization Form
  An adult who wishes to participate in any adult volunteer position or activity must complete this form. (It only needs to be filled out once.) Provides emergency contact information.

  Use this form to register for adult training courses.

Girl Forms

• Girl Membership Registration Form
  Registration form that must be completed by new members and by current members at the start of each new membership year. Please return completed registration forms with $10 annual membership dues to the Girl Scout Center.
• Girl Scout Authorization Form
  Form for parents to fill out and return to their leader before their daughter(s) may be registered as a Girl Scout. Provides emergency contact information.
• Financial Assistance Application (Promise Fund)
  To help a girl enjoy the benefits of Girl Scouting when a parent or troop is unable to provide the necessary funds.

• Cookie Ambassador Position Description and Application
  Cookie Ambassadors act as spokespersons for the Girl Scouts of the Golden Plains Council during the annual Girl Scout Cookie sale at group events or during media interviews.

Volunteer Recognition Forms
Nomination forms for volunteer recognition awards that are handed out at the annual Volunteer Appreciation Dinner and at service unit and troop meetings. Please note submission deadlines.

• Nomination Forms for Service Unit and Council Recognition
  Must be filled out to nominate an adult volunteer for a service unit or council award (lists service unit and council awards volunteers may be nominated for).
• Volunteer Recognition Profile
  Volunteers need to fill out this form, with one copy sent to the service unit and another to the Girl Scout Center to be kept on file. This profile lists any awards a volunteer has received as well as his or her length of service.

• Junior Girl Scout Badge and Sign Record Sheet
  A list of badges that are available to Junior Girl Scouts and the activities that are required to earn them. This form helps girls keep track of the activities they have completed.
• Permission Slip
  Used for all troop outings. Form is filled out by leaders, the copied and sent to each girl's parent(s) to give them specific information about an upcoming event, trip or activity. Parents must sign and return the bottom portion of the form to authorize their daughter's participation.
• Request To Travel Out-of- Council
  A sheet that explains the age guidelines and procedures for planning an out-of-council trip; includes the form that must be sent to the Girl Scout Center for approval.
• Teen Girl Scout Interest Project Record Sheet
  A list of Interest Project Patches that are available to Teen Girl Scouts and the activities that are required to earn them. This form helps girls keep track of the activities they have completed.
• Troop Finance Summary: | Editable Excel Sheet | pdf format |
  Use this form to report your troop's income and expenses each year. Please note: This form must be filled out in triplicate. Keep one copy for yourself and send two copies to the Girl Scout Center.

  Used by troops and service units to report non-monetary sponsorships
• Volunteer Interest and Talent Survey
  Forms that are handed out at parent meetings; helps leaders get an idea of what general tasks or activities parents may be willing to assist with.
